Admissions Information 2026–2027


St. Patrick Catholic School admission requirements for VPK through Eighth Grade are as follows:

All Families Must Submit:

  • Completed Application Form
  • True copy of legal papers (i.e., custody arrangements, if applicable)
  • Original Birth Certificate (to be copied and returned)
  • Baptismal Certificate and First Communion Certificate (if applicable)
  • Psychological/Educational Evaluations (if applicable), including but not limited to:
  • Individualized Education Plan (IEP)
  • Section 504 Plan
  • Comprehensive English Language Learning Assessment (CELLA)
  • Progress Monitoring Plan (PMP)
  • Student Support Plan (SSP)
  • Speech, Language, Occupational, etc. Evaluations

 Kindergarten: Florida Diagnostic and Learning Resources System (FDLRS) or Early Learning Coalition (ELC) Childhood Developmental Screening Results


VPK – Four-Year-Old Program Requirements

  • Must be four (4) years of age on or before September 1, 2026
  • Must be fully toilet trained

Kindergarten Requirements

  • Must be five (5) years of age on or before September 1, 2026
  • Report card from any previous educational program

First Grade Requirements

  • Must be six (6) years of age on or before September 1, 2026
  • Successful completion of kindergarten
  • Report card from any previous educational program

Second Grade Through Eighth Grade Requirements

  • Report cards from the last two completed academic years, including the most recent report card
  • Standardized test scores from the last two years (if applicable)

Enrollment Requirements


We require Health Forms HRS-680, Part A, B, or C and HRS-H Form 3040 prior to attending school.

Florida law states that no student may be admitted until St. Patrick Catholic School has received these forms. The Diocese does not accept Religious exemption.
